Sinus bradycardia fulfills the criteria for sinus rhythm but the heart rate is slower than 50 beats per minute. Sinus bradycardia occurs on an ECG when there is a normal upright P wave in lead II ― sinus P wave ― preceding every QRS complex with a ventricular rate of less than 60 beats per minute. A slow heart rate can also be a major contributor to angina. An ECG is therefore an essential part of the investigation of any patient with a slow pulse rate, and indeed of any patient with dizziness, syncope or breathlessness.
